Below are some of the most frequently asked questions patients have about our office policies and procedures.  If you have any other questions, or would like to schedule an appointment, we would love to hear from you.

A: Office Hours begin at 8:00am for patients with appointments.  We do not take walk-ins.
A: Yes.  We ask that you cancel your appointment 24 hours in advance. This will allow us to scedule another patient into that time slot. There is a $25 fee for late cancellations and a $50 fee for no-show appointments.  The bill will be collected prior to another appointment being scheduled.
A: You will be sent new patient paperwork when you or your referring doctor schedules your appointment.  We ask that you bring that paperwork filled out and arrive 30 minutes prior to your appointment time. The Front Desk will review the paperwork and help you with any questions you may have.  In addition, please bring your insurance card and a photo I.D.
A: Parking is free at all four of our clinic locations.  At our Main Clinic location on Eastmoreland there is a parking garage at the corner of Cleveland and Linden, with a walk-thru to our offices.  You will be given the parking code to exit the garage at the completion of your appointment.  There is general parking at all three of the other clinic locations.

Prescription requests should be called in Monday through Friday between the hours of 8:30am and 3:00pm.  Requests made after 3:00pm may not be phoned in to your pharmacy until the next business day.  Please have the pharmacy phone number readily available.  If you are asked to leave a message, please leave your full name and either date of birth or last four of your social security number.

The doctors do not have copies of your medical records after hours or on the weekends.  For this reason, prescriptions and refills are only issued during office hours.

A: Telephone hours are 8:30am - 4:30pm Monday through Friday.  If you experience an emergency, always call 911 first.  If it is necessary to contact a doctor on call after hours, please call the main number, 726-5300 and leave a message with the answering service.  The call will be returned as soon as possible.
A: There are charges for copying medical records and completing forms like Family Medical Leave Act paperwork.  The fee is dur prior to the release of the information.  Please call our office and ask for the Medical Records Department for more information.

Insurance claims are filed as a courtesy to our patients.  

We are enrolled in numerous insurance plans and we will file with your carrier.  However, it is your responsibility to understand the requirements and guidelines of your plan regarding labs, diagnostic facilities, physical therapy centers, surgical centers and hospitals that your plan covers. 

If a claim is disputed or denied by your insurance company, you will be responsible for that payment.
